The most convenient way to show your vaccinated status! Premium double-sided CR80 PVC cards fit perfectly in your wallet, purse, or around your neck!

Here's how to get your VaxCard:

• Choose a card theme from the dropdown menu
• Choose if you would like a lanyard from the second dropdown menu
  
• Upload your government-issued vaccination certificate where requested
• Upload a headshot photograph of the individual for which the VaxCard is to be created (Please present a front-facing photo from the neck up on a neutral background)

Important Notice For All Individuals Vaccinated in other countries:

You are still eligible to receive a VaxCard. Please upload the document (scan or take a photo) given to you by an official vaccination center in the country it was administered. Continue the process as usual. QR Codes cannot be created unless uploaded by the individual. 

Orders placed by 11am are available for next-day pickup between the hours of 10am to 3pm. Orders placed after 11am will be available within 48hrs. All weekend orders are processed on Monday and will be available for pickup within 48hrs.
